Dr. Langley is an out-of-network provider with all insurance companies. Payment in full, by cash or credit card, is expected at the time of service. As a courtesy, the office will provide you with all information needed so that you may submit a claim for direct reimbursement, depending on your plan coverage.
*Please give 48 hours notice to cancel an appointment in order to avoid being charged the full fee for the appointment.
*Monday appointments must be cancelled by 5pm the Thursday prior. The office is closed on Fridays. Cancellations with adequate time allow me to schedule patients who are waiting for an appointment.
*A valid and active Credit Card number is required to be on file at the time of appointment confirmation. Your credit card will be charged after each appointment.
